Help America Hear Program


The Foundation for Sight & Sound turns dreams into possibilities by helping those with vision and/or hearing loss to learn, live, and work better.

The Foundation for Sight and Sound in partnership with the EarQ Group has developed the Help America Hear Program to provide hearing aids for men, women, and children with limited financial resources.

In most states, Medicare and Medicaid do not cover these costs and many insurance providers do not offer full medical coverage.

The manufacturer suggested retail cost averages from $1,500.00 to $3,000.00 per hearing aid.  With this program, each person receives: two hearing aids, custom ear molds, one year limited service, and a one month supply of batteries.

Help America Hear is sponsored by the generosity of hearing health care providers, corporate and private donors, and manufacturers.

We would like to especially thank our hearing health providers for donating their time and effort, and most importantly, for giving our Help America Hear patients the commitment and dedication they deserve.

We trust that you will deeply appreciate the kindness of all those funding this program, and as a courtesy to them, we ask all applicants to ensure their financial eligibility.

Help America Documents - FAQs:

1. What is the "Help America Hear Affidavit?"

The HAH Affidavit is a document that needs to be signed and notarized by the applicant. This document guarantees authenticity for all financial documents submitted to our program.

2. What is the "Patient Hearing Aid Form?"

The "Patient Hearing Aid Form," is a document that certifies the physical wellness of the applicant.

The form provides TWO options and the applicant MUST pick one :

He or she can have his/her general physician sign the 'medical clearance,' section (option 1) OR the applicant can sign and WAIVE his/her right to receive a physical prior to receiving hearing aids (option 2).

3. What is the "Photo-Video Release?"

The "Photo-Video Release," is a document that gives The Foundation for Sight and Sound the right to contact any approved applicant at any time. It, also, gives us permission to use the testimonial or personal story of an (approved) applicant for any future marketing or public relations campaign.

 

Help America Hear Program - FAQs:

1. How does someone apply for a hearing aid?

In order to apply, you will need to include all the following documents:

Patient Application, Proof-of-Income, Audiogram, Photo-Video Release Form, Signed & Notarized Affidavit, Medical Clearance, and the non-refundable $75 PER hearing aid application fee.

2. Who is eligible for a hearing aid?

Those who make less than $25,000 and have aid-treatable moderate to severe hearing loss (as determined by a Specialist)

3. How long does it take for approval for a hearing aid?

The entire application process takes anywhere between: 30-90 days


4. What is the degree of hearing loss to be eligible?

Moderate to severe as determined by a Specialist


5. What types of hearing tests are required?

An Audiogram Exam


6. Who pays for the audiogram?


You are responsible for the cost of the hearing test. Although, many insurance companies do not cover hearing aids, several do cover testing. If you have insurance coverage currently, call the number on the back of your insurance card.

7. Who contacts the hearing aid provider?


Once your documentation is reviewed by our committee and your approval is finalized, we will contact you with the information of an Ear Q provider nearest your home. The provider will, then, schedule your fitting, accordingly.

8. If I don’t have a tax return what other income proof can I provide?

Social Security or Social Security Disability Year-End Statement

In cases where medical expenses exceed income, receipts are required
